We are pleased to offer the opportunity for free sport physicals to families of Iron Mountain Public School students going into the grades 7-12 for the 2024-25 school year. In partnership with UP Rehab, Marshfield Medical Center Dickinson will be hosting the event on campus this upcoming April 24. Families must sign up using the link provided above.

IMPORTANT
** On-Site providers are not obligated to provide clearance to any student-athlete. If the provider finds any issues, either with the medical history or during the examination, they may require that the student obtain clearance from the primary care provider, or be able to provide additional documentation. If your student has a history of heart issues or other significant medical issues, it may be beneficial to obtain a physical through the student's primary care provider.**
**Physical card MUST be filled out and signed by parent/guardian prior to physicals. Students WILL NOT be able to receive a physical if this is not done.** This card can be printed from the link below or picked up in the IMPS front offices.

Please note that attendance at school physicals does not guarantee the physical card will be signed. This decision is solely up to the medical provider performing the examination.
General Information

Who:
Any Iron MountainHigh School or Middle School student who will be in grade 7-12 and participating in Iron Mountain sports for the 2024-2025 school year. Unfortunately, we are unable to offer these physical dates for students from other schools or youth athletes.
Where and When:
All physicals will be offered at Iron Mountain High School. Students will enter through the high school gym doors.

Cost:
Sports physicals are FREE.
Required Documentation:
MSHAA Physical Card. This card MUST be filled out and signed prior to arrival at the school. Limited quantities of the cards may be picked up at the Iron Mountain High School office or may be printed from the link below. Physical cards will not be considered valid until the entire form is filled out and all required signatures have been obtained. Physical cards will not be permitted to leave the school once the medical provider has signed.
